1st day of class-Monday, April 2 at 10 am
Email us at affinityhealthcarega@gmail.com by March 13, 2018 and we will send you a packet.
Please bring COMPLETED packet, your driver's license, and your social security card to the first class.
If you don't have a driver's license, please have a photo ID. You will need a social security card to enter into testing at the end of this course. If you don't have a social security, please visit the Social Security Administration. It takes about 2 weeks for your card to come back.

Open house this Saturday 9/3/16 and Monday 9/5/16 from 2-4pm and Wednesday 9/7/16 from 8-10am at Affinity Healthcare Training Center 187 Roberson Mill Rd Suite 107. Information packets will be distributed at those times. All those who visit the training center will be entered into a drawing for a gift card.
