Time flies. 8 years ago we graduated from our Orthopaedic surgery residency. Since then, the world has changed significantly. Our chairman has passed, along with some great colleagues. I still consider these 3 guys my brothers. All practicing in different parts of the country, yet still tied to metro Detroit. This is home!

Mountain bike accidents are super common in western NC. This patient suffered a distal 1/3 clavicle fracture after a bad wreck. These injuries have a good chance of not healing and often need surgery. Repaired this fracture with a superior plate, then scoped the shoulder and placed a metal button under the coracoid to increase the fixation strength.

Another interesting case this week. This patient had symptoms consistent with bicep tendinitis and had failed conservative treatments. Arthroscopy of her shoulder revealed a few uncommon bicep issues. First, her bicep had split into two attachments onto the glenoid. Also, one of the limbs had completely scarred into her superior capsule/rotator interval tissue. She also had a Buford Complex, which is a more common anomaly. Had to free up the bicep, then performed a “loop and tack” style arthroscopic tenodesis to fix the issue.

Calcific Tendonitis is a common shoulder issue where calcium deposits form usually within the rotator cuff tendons. After failing a course of conservative management, this patient underwent surgery to fix his issue.
1) XR image with calcification circled in red
2-3) MRI images showing the deposits near the rotator interval and anterior cable supraspinatus
4) intraop image with probe identifying underlying calcification
5-7) calcific areas identified by red arrows
8 ) defect in area was large enough that a repair was performed of the remaining healthy tendon

Not a case I see often, but interesting and a relatively common post op complication.
This patient had her shoulder replaced by a different surgeon. When she started having pain a few months after the procedure, she saw me for a second opinion. She was losing motion in her joint and getting pain over her posterior shoulder. Initial XRs showed a subtle non displaced fracture of her acromion, a known complication following Reverse Total Shoulder Arthroplasty. After a long period of conservative care, including bone stimulator usage, the fracture had not healed. Eventually we decided to go and fix the fracture. Two plates were placed onto the scapula to stabilize the fracture, along with bone graft.
