Aviva Romm, MD September 11 at 2:42 PM · A nuchal cord (or cord-around-the neck) is when the umbilical cord is wrapped, 360 degrees, around the baby’s neck. If you’ve been pregnant, you know just how much babies move around in there. Sometimes it can feel like they’re doing somersaults and backflips and that’s because - they are! And this movement is how the cord can get wrapped around the neck. Occasionally a nuchal cord can occur because of a low amount of Wharton’s jelly, the proactive gel in the cord, and it may be more likely to occur in twin pregnancies due to excessive amniotic fluid or a very long cord. I’m mentioning this variation of normal because nuchal cords are often cited as the sole reason for a cesarean section , and over my years of midwifery and medical practice, I can’t tell you how many times I’ve heard someone say homebirths are dangerous because baby could have a cord wrapped around her neck. But here’s the thing: nuchal cords are common. Very common. In fact they occur in 37% of full term births. They aren’t preventable, but complications are extremely rare, and they are almost never life-threatening. Plus they aren’t usually detected until baby is being born because they’re hard to see on ultrasound and don’t usually cause problems or symptoms that your provider can detect. If baby is born with a nuchal cord, your midwife or doctor will gently unwrap the cord from around baby’s neck with her middle and index finger as soon as the head is past the cervix. Of course, if there are any issues with baby’s heart tones in labor - which could be due to a short cord or the cord wrapped around other parts of the body as well - you’ll take the necessary steps you need to for baby’s well-being. Sometimes heart tones go down in labor and there’s a diagnosis of fetal distress leading to cesarean. But I wanted to put this issue to rest because it really is a variation of normal, not usually an emergency. And as you can see in this birth video above, mama does what midwives do - we unwrap the cord as the baby is born - and rarely is more needed than that. Stories to share about a cord around the neck? Have you tried dates? It’s on our list we hand to clients. Aviva Romm, MD August 18 at 2:00 PM · Dates have been used around the world for centuries as a natural approach to preparation for labor. Several studies now support their effectiveness. Though we don’t know exactly why, they do have an impact on the start and progress of labor - likely due to an impact on the hormone oxytocin which helps labor start and progress effectively.⁠ ⁠ A 2011 study found that women who ate dates starting at 36 weeks were:⁠ 🔸 Significantly more dilated when they got to the hospital⁠ 🔸 Had a significantly higher rate of intact membranes⁠ 🔸 Were significantly more likely to go into labor spontaneously (i.e. without induction)⁠ 🔸 And had nearly half the length of first stage of labor⁠ ⁠ A 2014 study found that women who ate dates from 37 weeks on had:⁠ 🔸 Greater cervical dilatation at admission⁠ 🔸 Higher success rates of labor induction when needed⁠ ⁠ A 2017 study concluded that all the above was accurate and yet another study found that eating dates in pregnancy led to less bleeding immediately after birth.⁠ ⁠ Any type of dates work! The ‘dose’ is about 2.5 ounces of dates ( ~6 to 8 small dates or ~4 larger dates) daily, starting at 36 or 37 weeks and until labor begins. I love eating them plain, but you can include them in my “herbal bliss bites” (recipe on my IGTV), use them to sweeten cashew or almond milk, or include them chopped in yogurt or oatmeal. We’ve decided to take a good look at breastfeeding promotion and how it is also performative, promotes shame, perfectionism, and low self esteem for many new parents. Lactation books and education for new parents tout the benefits of breastfeeding. And I have seen lactation professionals demonize formula and commit to “educating people about the risks of formula”. In a system that shoos birthing people away with “see you in six weeks” after being cared for by a nurse who has never helped anyone lactate long term post-colostrum, folks often return home to criticism from friends, elders, and more breastfeeding promotion with no accessible support. Sometimes, their instincts tell them to feed their baby formula. And often times, they are actually right. People who feed their babies formula out of love for the baby and THEMSELVES aren’t worse parents. Their babies aren’t less healthy. When the alternatives are starvation, dehydration, or desperately deteriorating mental health, formula is an option that makes a hell of a lot of sense. You’re not better because you never gave your baby formula. Just because a baby had some formula it doesn’t mean they can’t nurse or the whole thing is ruined. In fact, more than half of women have bre@sts that are different volume, shape, position, and size. The human body isn’t perfectly symmetrical. But the invention of photoshop has left generations of us thinking it all should be. Other things that are normal: ✨Different nipple sizes ✨Wider set ✨Narrow set ✨Cone shaped ✨Bell shaped ✨Large areola ✨Small areola ✨Areola that don’t match ✨Extraverted, introverted, & extra nipples You get the idea. If you’re carrying fear, stress, worry, anxiety into your birth it will likely have an impact on the way you are perceiving pain. # So yes do the stretches and eat the right things but make it a practice to practice relaxing your mind and releasing the emotions that are introducing tension into the body.

Let’s talk about Vitamin D and Chestfeeding: ☀️ Am I passing enough Vitamin D along to my infant? ☀️ Do I need to directly supplement my infant with Vitamin D? ☀️☀️ANSWER☀️☀️ According to this PubMed article, if the lactating parent supplements with 6400IU/day, they can adequately supply their chestmilk with enough Vitamin D for their nursing infant and is an alternate strategy to direct supplementation. It matters to students and Residents. It matters because otherwise ‘we’ as a collective healthcare institution traumatize, retraumatize, disempower, shame, harm, inflict pain, assault, and ostracize people from their healthcare, from their health, and from our care. Enough. Enough. Enough. No healthcare rationale can ever justify forcing care on a patient, ie assaulting a patient. I’m done making space for that argument. Let’s talk about Vitamin D and Chestfeeding: ☀️ Am I passing enough Vitamin D along to my infant? ☀️ Do I need to directly supplement my infant with Vitamin D? ☀️☀️ANSWER☀️☀️ According to this PubMed article, if the lactating parent supplements with 6400IU/day, they can adequately supply their chestmilk with enough Vitamin D for their nursing infant and is an alternate strategy to direct supplementation. Here are some exercises you can do to relieve tailbone pain. MamasteFit & The Doulas of Mamaste is at MamasteFit & The Doulas of Mamaste. March 12 at 6:38 AM · Aberdeen, NC · The coccyx, or tailbone, is the tip of our sacrum and supported by various ligaments and musculature of our pelvis and pelvic floor. We can experience pain if the muscles are too tight and don’t allow the sacrum/coccyx to move, or even pull it into a position that’s out of alignment. We can also experience it if the opposite is occurring, the muscle that support the coccyx are too weak or loose and they cannot help stabilize. When doing these exercises, focus on feeling a stretch in the back part of your pelvic floor. Inhale down and out, feeling the pelvic floor expand and lengthen, then exhale to relax completely! Identifying which could be causing your pain can be done by an individualized assessment from a physical therapist! We have two pelvic floor PTs in our gym: @hayleykavapt and @themissionpt! Learn more techniques on how to relax and prepare your pelvic floor for birth with Dr Brooke Wiltshire of @themissionpt this Sunday March 14th at 12pm EST!
