Bimaxillary advancement for obstructive sleep apnea
A 10mm advancement measured at the upper incisor tip and 11 measured at pogonion gave a volume airway increase from 16.5cm3 to 33.3cm3 and a minimal cross sectional area increase from 145mm2 to 470mm2.
Also, he is able to sleep on his back without CPAP and wakes refreshed without daytime sleepiness. PSG pending.
Orthognathic surgery airway changes: the maxilla advanced 6mm at the incisor, 12mm at pogonion. As a result, the minimal cross sectional airway (the smallest portion of the airway) expanded 10x’s the original, the nasal/oropharyngeal volume of the airway expanded over 3x’s the original . #airway #orthognathic #mma #jawsurgery

When God gives you small laterals: tales of arch length discrepancies.
Class II patients often have small laterals and the resultant arch length discrepancy needs to be managed if proper overbite is desired. Most often spaces distal to the laterals are requested which can later be restored. Sometimes when the DL spaces are not provided, the multisegment LeFort can be done between the canines/premolars and the central incisors. A diastema can then be created which handles the arch length discrepancy and can later be distributed across the anterior for esthetic restoration.
This was today's surgery where the diastema was opened (pogonion advancement of 19mm)
