Beacon Medical Group Centennial Neighborhood Health Center is a patient-centered medical home, a team-based model of care led by a primary care physician, social worker, clinical pharmacist, nurse/medical assistant and case manager, who together provide continuous and coordinated care to improve the health of you and your family throughout your lifetime. Our team will address all of your health care needs, including referrals to other medical professionals.
We accept Medicaid, Medicare, private insurance and offer a sliding fee scale based on income and federal poverty guidelines for the uninsured. In addition, we offer a special cash discount program for our patients who do not have insurance or do not qualify for other assistance. Our clinic is handicapped accessible and provides interpretation services.
