Dear Family and Friends, On 09/22/20, we completed full house testing of all Care Center Residents, Bridgeway Estates Tenants and all Staff. Our top priority is to keep everyone safe and healthy. We are collaborating with our Medical Director and the Minnesota Department of Health and are following all recommendations from the Centers for Disease Control & Prevention and the Centers for Medicare & Medicaid Services to ensure we are taking all appropriate steps at this time. This testing identified one new staff positive and the results allow us to accurately isolate the virus and prevent the spread in our community. We believe this is the best way to protect residents, staff and our community and track the path of the virus. Testing results are shown below. There were no Care Center Residents or Bridgeway Estates Tenants who tested positive. We are still waiting finals results on some staff. We will test all residents and staff on 9/29/20. Bridgeway tenants will not need to be tested. Our staff are equipped with specific training and PPE to continue to provide safe, quality care, services and support to the residents of our community. As we’ve shared several times, in early March, we initiated proactive infection control practices, screening procedures and visitor restrictions that will help us manage this situation as effectively as possible and protect the safety and health of all who live and work here. Dear Family and Friends, On 10/06/20, we completed entire-site testing of Little Falls Care Center residents and staff. The results are shown in the chart below. We have had 2 additional staff members and 1 contracted staff member test positive for COVID-19. At this time, NO residents have tested positive. We will test all residents and staff at Little Falls Care Center on 10/13/2020. Following the Centers for Medicare and Medicaid Services (CMS) and Minnesota Department of Health (MDH) guidelines, we are now required to test our care center staff twice weekly. We’ve recently had to open our COVID unit. It’s not been easy, but, we persevere. We do this for those who can’t. We do this for those who have entrusted to us, their most sacred possession; their loved one. And not only will we not let them down, we won’t let you down. And as important as that is, equally important is that we won’t let each other down. Now is the time where our teamwork, our commitment, our dedication, and our utter resolve to never give up will shine. Face Everything And Rise. Our priority as COVID-19 continues to spread across our state is to keep everyone who lives and works here safe and healthy. We are realistic about the challenges we face in combatting COVID-19 and have appreciated your support during the long months we’ve endured so far. As you know, we have residents and staff in our community who have tested positive for COVID-19. We tested staff on Friday, October 30th, and received an additional positive staff member from this testing. We are testing all residents and staff at the care center again on Tuesday, November 3rd and all staff on Friday, November 6th. We will continue to routinely test staff at LFCC twice weekly due to our high county positivity rate. Bridgeway Estates have received all negative results and are now done with quarantine and outbreak testing. Due to the current county positivity rate, we are unable to initiate indoor visiting at this time. Bridgeway Estates has started communal dining during lunch. The residents who have tested positive for COVID-19 are isolated from the rest of our community and are being cared for as they recover by a designated staff team. The staff who have tested positive are quarantined at home and will not be allowed to return to work until they have recovered from the virus and passed our return-to-work protocols. We continue to work with the Minnesota Department of Health and are following COVID-19 recommendations from the Centers for Disease Control & Prevention and the Centers for Medicare & Medicaid Services. Our top priority is to support and protect our residents and dedicated caregivers. We have equipped our staff team with the training and supplies needed, such as surgical masks, eye protection, gloves and gowns, to provide safe, quality care, services and support to our residents. We have worked with everyone in our community on hand hygiene and safe physical distancing.

We celebrated two more residents beating Covid-19!! They graduated off our segregated unit and were greeted with staff and cupcakes. 🎉
Today was a momentous day at LFCC. 40 days after we opened our COVID-19 unit, we were able to celebrate the graduations of two residents and close the unit down! 🎉 We are so grateful for the staff who volunteered to care for residents inside the unit. Please join us in thanking Michelle Moebakken, RN; Stacy Sickles, LPN; Donnie Anderson, RN; Karen Johnson, LPN; TJ Mason, TMA; Carly Lathrop, RN CM; and Stephanie Orpen, TMA for their selflessness, hard work, and dedication. Thank you to every single team member of LFHS for your support and perseverance throughout this pandemic. We may have closed our unit, but COVID-19 is still a threat to our residents, tenants, and staff. Morrison County has seen record numbers of new cases in the past weeks. Tonight, we’re kicking off a new myth buster series to set the record straight: ❌ Myth: COVID-19 vaccines are not safe because they were developed and tested quickly. ✅ Fact: Many pharmaceutical companies have invested significant resources into developing COVID-19 vaccines quickly because of the worldwide effects of the pandemic. This emergency situation warranted an emergency response. That does not mean the companies bypassed safety protocols or performed inadequate testing. Mayo Clinic will recommend the use of those vaccines that it is confident are safe. While there are many COVID-19 vaccine candidates in development, early data are encouraging for the Pfizer vaccine, which likely is to be the first authorized for emergency use by the FDA. This vaccine was created using new technology based on the molecular structure of the virus that allows it to be free from materials of animal origin and synthesized by an efficient, cell-free process without preservatives. This vaccine developed by Pfizer/BioNTecH has been studied in approximately 43,000 people. We appreciate and support the decision made by the federal government to prioritize long-term care residents and staff in the distributions of the COVID-19 vaccine. Information on our gift giving tree! Friends and family, are you looking for ways to help us or donate? Here a wonderful way to bring joy to our residents. We have made up gift tags with ideas for our residents in order to bring some happiness this Christmas season. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). • Little Falls Care Center – all negative staff and residents • Bridgeway Estates – all negative staff and residents We are excited to announce that we are taking actions to distribute the 1st of the two-step COVID-19 vaccine. Staff will reach out to residents and/or family members to get consent in the coming weeks. There will be no cost to our residents or staff for the vaccine. Doses will be administered on site, 21-28 days apart. If a resident or staff person has previously tested positive for COVID-19, it is still recommended they be vaccinated. Getting vaccinated against COVID-19 is one of the best ways to protect yourself and everyone around you, particularly those who are at increased risk for severe illness. Wearing masks and social distancing help reduce your chance of being exposed to the virus or spreading it to others, but these measures are not enough. Getting the vaccination will not only protect you from being infected; it will help us reduce visitation restrictions in the future. In the meantime, we urge you to practice social distancing, mask use, and staying home when possible. Please refrain from taking your loved one home for the holiday. If a resident does choose to go to a holiday gathering outside of our care center, the resident will need to quarantine in his or her room for 14-days after returning as a necessary precaution. Staff are also asked to only celebrate with members of their household and to follow all quarantine guidelines after traveling. The news of a vaccine gives us hope. I worked at the paper mill here in town for 40 years which was only supposed to be a temporary position. While I was stationed in Alaska my brother asked me if I would come to Minnesota to be in his wedding. I was put on the reserves and came home for a short stay. When I was here I was asked to fill in at the paper mill for a guy that was going on leave for 30 days. He never came back so I remained there ever since.

The past 2 Monday's have been very insightful for the Leadership Team here at LFHS. On the 7th, our Dietary Manager presented to the group the Simon Sinek video she watched on Millennials in the workplace about what she learned, how it impacted how she views things in her own department and how she will take what she's learned and apply it to how she leads. The "one word" she chose to write on her puzzle piece? Confidence. This week, it was our Director of Nursing presenting on the video she watched on empathy, what she learned and how she will apply it to how she leads. Dear Family and Friends, We will be transitioning to test staff once weekly. Morrison County has seen a large decrease in cases, and the positivity rate remains below 10%. Results from testing this week is shown below. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). • Little Falls Care Center – One new positive staff • Bridgeway Estates – All negative tests We are excited to announce that our care center is scheduled to receive the Moderna COVID-19 Vaccine and will host 3 clinics for vaccinating our residents and staff! Our first clinic is 1/5/21. The second dose of the vaccine will be administered 4 weeks after the first round and the third will be administered 4 weeks after the second. Protection from vaccine is not immediate; vaccine is a 2-dose series and will take 1 to 2 weeks following the second dose to be considered fully vaccinated. Vaccine efficacy is 94-95%. This data was assessed with two doses being administered. It is very important to get both doses to ensure good protection. Assisted living vaccination dates will be sent out as soon as we receive them. When someone receives the vaccine they may experience side effects up to three days after vaccination. These side effects include injection site pain, tenderness and swelling of the lymph nodes in the same arm as the injection, swelling (hardness), and redness. General side effects include fatigue, headache, muscle pain, joint pain, chills, nausea, and low grade fever. Getting vaccinated against COVID-19 is one of the best ways to protect yourself and everyone around you, particularly those who are at increased risk for severe illness. Wearing masks and social distancing help reduce your chance of being exposed to the virus or spreading it to others, but these measures are not enough. Getting the vaccination will not only protect you from being infected; it will help us reduce visitation restrictions in the future. While the vaccine is a significant factor in our fight against the virus, it is just the beginning to reopening our care centers. We continue to use the county positivity rate to measure the spread of the virus in our local communities and follow the recommendations from MDH and CDC on reopening. Our county positivity rate has decreased significantly, but the care center cannot begin indoor visitation until we have gone 14 days with all negative test results. Bridgeway Estates will begin indoor visits on Tuesday 1/5. Visits can be scheduled by calling either Krista or Sam at 320-632-2062 or 320-632-2089. Visitors must be able to produce a NEGATIVE Covid-19 test from 2-3 days prior to the initial visit or a positive test within the last 90 days as long as the individual is out of their infectious period. Team member Sharon Freudenrich, who has been eagerly awaiting the COVID-19 vaccine says, “I am so happy I got the vaccine because I am finally helping to stop this pandemic!” Regarding Assisted Living buildings, the MDH, Preferred Pharmacies and the Public Health Departments are currently working on a plan to start vaccinating tenants and staff. We are anticipating vaccines to be administered at Bridgeway Estates in the coming weeks. Dear Family and Friends, On Friday, January 15th, we had one care center staff member test positive for COVID-19. We will be testing all staff and residents again on 1/19/21. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). As a reminder the staff who test positive will be quarantined at home and will not be allowed to return to work until they recover from the virus, are symptom free and pass the employee screening protocols. Our staff are equipped with specific training and personal protective equipment (PPE) to continue to provide safe, quality care, services and support to the residents of our community. All tenant and staff test results from Bridgeway Estates’ testing on Friday 1/15 were negative. They will test again on Wednesday 1/20. Due to the recent positive cases at LFCC and BWE, we must continue to refrain from hosting indoor visitation at this time. We are collaborating with the Minnesota Department of Health and are following all recommendations from the Centers for Disease Control & Prevention and the Centers for Medicare & Medicaid Services to ensure we are taking all appropriate steps at this time. We are testing all tenants and staff for COVID-19 on 1/15/21. To ensure the health and safety of our tenants and staff, we will discontinue meals in the dining room and indoor visits. Any exposed or symptomatic tenants will be quarantined in their apartment. Our vaccination clinic on 1/19 will proceed as scheduled. Any tenants contagious for COVID-19 will be vaccinated at our second clinic in February. Any staff who test positive will be self-quarantined at home and will not be allowed to return to work until they recover from the virus; 10 days following positive test and 72 hours from last fever without using medication. Our staff are equipped with specific training and PPE to continue to provide safe, quality care, services and support to the residents of our community. Dear Family and Friends, We are transitioning to testing care center staff bi-weekly. Results from testing this week is shown below. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). • Little Falls Care Center – all negative results As of today, the MDH is not recommending any changes to our visitation process. Currently we are offering virtual visits and indoor visits at our designated visiting area. Since the start of the pandemic, 3,977 residents of Minnesota's long term care facilities have died from COVID-19, accounting for 63% of the deaths from the virus statewide. However, last week, Minnesota recorded an average of just 64 new coronavirus cases per day in long-term care settings; the lowest the state has experienced since September! On Tuesday, MDH released more data showing that overall trends are improving, and not just in senior services communities. The state's positivity rate of testing has fallen to 4%, the lowest rate since June! The rate had peaked at 15.5% in November amid an upsurge in cases. The drop in new cases is a hopeful sign that the vaccines are starting to provide the protection we need and the infection control protocols put in place are working. Our staff continue to find creative ways to ensure social distancing does not stop social interaction. This week we plan to celebrate Valentine’s Day by having a party, activities, and crowning two Valentine’s Day Royalty. This week, the Minnesota Association for Geriatrics Inspired Clinicians (MAGIC) launched a new video resource to help providers in our community to address hesitations and misinformation related to the COVID vaccine. MAGIC COVID Vaccination Video was developed to provide accurate and up to date information and dispel myths regarding the vaccine. The goal is to increase vaccination rates, specifically among frontline health care workers, by providing meaningful information from a trusted source that allows them to make an educated decision. Our care centers next vaccine clinic is on March 3, and Bridgeway’s next clinic is February 16th. Looking for ways to help? We continue to receive questions on how and when family and friends will receive their vaccine. Health officials have created a Vaccine Connector tool found at https://vaccineconnector.mn.gov/. This tool helps Minnesotans find out when, where, and how to get a COVID-19 vaccine. After you fill out a simple form, the Vaccine Connector will alert you when you are eligible to receive a vaccine, connect you to resources to schedule a vaccine appointment, and notify you if there are vaccine opportunities in your area. It is not necessary to sign up if: • You have already received your COVID-19 vaccine. • You previously registered for the COVID-19 Vaccine Pilot Program or the COVID-19 Community Vaccination Program. You’re already on the Vaccine Connector list, and there is no need to re-submit your information. • You completed the Minnesota COVID-19 Vaccine Education and Child Care survey. You’re already on the Vaccine Connector list, and there is no need to re-submit your information. • You completed the Health Care Connection Form, you do not need to re-submit your information. Dear Family and Friends, We are continuing to testing care center staff bi-weekly, and we will be testing Tuesday, March 9th. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). This Saturday, March 6th, marks one year since Minnesota confirmed its first case of COVID-19. The state has since recorded 485,230 total confirmed or probable cases. Now, more than 52% of Minnesotans 65 and older have received at least one dose of either COVID vaccine. This week, Governor Tim Walz stated Minnesota will expand vaccination eligibility when 70% of that population gets their first dose. Vaccine eligibility would expand to people with high-risk health conditions and targeted essential workers. Younger groups and the general public would follow in the late spring and summer. Johnson & Johnson has now received an emergency use authorization (EUA) from the U.S. Food and Drug Administration (FDA) for its COVID-19 vaccine, the Janssen Ad-Vac. The J&J vaccine is different than its Moderna and Pfizer 2-dose vaccine counterparts in two ways. First, the J&J vaccine is a one-shot vaccine, not a two-dose vaccine. Second, the J&J vaccine uses a more common vaccine technology and does not utilize the mRNA vaccine delivery method. The vaccine is published as 85% effective 28 days post vaccination. Side effects listed for this vaccine are similar to those listed for Pfizer and Moderna vaccines.
