We wanted to let our patients know of a new process we are implementing here at Carpenter Dental Group. We are starting to send our health information and office policy forms out by text or email. When you receive a reminder text or email for your appointment please read the ENTIRE text because it will contain a link to click on to fill out your paperwork electronically. These forms are all encrypted and secure and governed by HIPAA regulations. These forms can also be accessed on our website at www.carpenterdentalgroup.com. Please click on the Forms tab and select the correct link.
Currently each patient is required by the ADA to fill out a prescreening form before each visit prior to their appointment. This screening form will be included in the text you receive 2 hours before your appointment. We are continually trying to improve our systems at Carpenter Dental Group. Filling these forms out online eliminates the handling of paperwork for our patients and staff in the office. We understand that some patients may be sensitive to sending certain information electronically. For these patients, we will have the forms on an iPad in the office for you to fill out when you arrive.
