Children’s Health Home Care provides family-centered, comprehensive care to children with chronic illnesses, as well as those recovering from hospitalization or injury.
We serve as a link between families, hospital providers and the patient’s primary care physician. Under a physician’s order and insurance authorization, Children’s Health Home Care supports a variety of conditions for children at home.
