The Community Rehabilitation Center is a nonprofit social services agency that offers behavioral and primary care, and community economic development services for under-served, low-income individuals with mental illness, substance abuse and HIV/AIDS who live in Duval, Nassau, Clay and St. Johns counties. More than 300 adults and families use CRC services at the center, in their adult-living facility, at home and elsewhere in the community every day. CRC is accredited with the Commission on Accreditation of Rehabilitation Facilities (CARF) and licensed by the State of Florida Department of Children.
