I do immediate implant placement pretty often, but usually not on molars. I find it difficult to get good placement. This case was from yesterday. It had divergent roots so I went ahead and placed it at the time of extraction after thoroughly cleaning the MB socket. Here is a fun case we did this morning. Immediate overdenture on the upper vs a partial lower denture. I always use a Conebeam and in this case 3 of the 4 implants were fully guided. We did run into trouble at one site as the bone was blown out on the buccal. Had to free hand that one but was able to get it pretty parallel with the others. No easy task on an upper!

So you’re never know sure when a surprise will show up during implant surgery. With a pre-op Conebeam I was somewhat expecting not to be able to place the 4 th implant on the top in this double arch All-on- X surgery. After the extraction of #11 and #12 there just wasn’t any bone. We had a trauma patient walk into the office this morning. Hit in the face by a piece of iron. ER had sutured his lip but didn’t deal with the broken maxillary bone. His front 2 teeth #8 and #9 we fractured deep in the bone and his buccal plate was literally crushed. The palatal bone was displaced palatally about 1 cm . We cleaned the wound, reduced the fractured, extracted the root tips, placed osteogenesis plugs and sutured him up. He looked way better!
