The Home Hospice Aide (HHA) is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well being, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.
DUTIES:
• Reports any unusual circumstance in the patient's condition or home environment to the primary RN/Patient Care Manager.
• Follows current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.
• As assigned, performs incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization.
• Completes a clinical note for each visit within required time-frames, which must be incorporated into the patient's record.
• Participates in the in-services program. Completes at least six hours of in-services every six months.
• Participates in daily report. Adheres to and supports care management model within the agency.
• Provides care according to the aide plan of care|assignment sheet and in accordance to State Practice Act or regulations.
REQUIREMENTS:
• Current CPR certification required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional AR State Specific Requirements:
• Required to meet at least one of the following: 1) at least 1 year of experience in an institutional setting (home health agency, hospital, hospice, or long-term facility) verified by a previous employer. 2) Certificate issued by the State of Arkansas for working in long term care facilities-copy must be available for review. 3) Completion of 40-hour aide training course meeting requirements of AR regulations.

Qualifications
Education:
Graduation from an approved school of nursing.
License:
Current license from the Arkansas State Board of Nursing.
Experience:
Minimum 7 years nursing experience.
Minimum 3 years hospice experience.
Certifications:
Current Basic Life Support CPR certification through the American Heart Association.
Other:
Working cell phone. Valid Arkansas driver’s license. Good driving record/reliable transportation. Must be able to lift and/or move more than fifty pounds. Frequently exposed to outside weather conditions. Physical demands are: push/pull/grip up to 50lbs., kneel and crouch as needed.
Preferred Experience:
5-7 years of home care/hospice clinical experience.
Preferred Other: Has understanding of the physical, psychological and spiritual concerns of terminally ill patients and their families. Good verbal and written communication skills. Good organizational skills. Good assessment skills with ability to make sound judgement. Ability to work with an interdisciplinary team.
