IMPORTANT INFORMATION FOR PATIENTS:
Please note that we have made some changes to our clinic processes due to COVID-19. Remember, when someone who has coronavirus coughs or talks, virus particles can spray from their mouth or nose. In order to minimize potential exposures, we are asking all patients to do the following:
1. BRING YOUR OWN MASK – Supplies are limited, so please bring your own mask, scarf, or bandana.
2. CHECK-IN BY TEXT – When you arrive in our parking lot, please stay in your car. Text your full name to ___number for your clinic____ and indicate if you are checking in for surgery or clinic. Our staff will respond within a few minutes.

3. GIVE US YOUR MEDICAL HISTORY BY PHONE – A team member will be contacting you by phone to discuss your eye issues prior to you entering the building.
4. MINIMIZE TALKING IN THE CLINIC – Your time with the doctor will be very focused on the exam and determining the best plan of care for you. If you have questions that may require a longer discussion, please arrange an additional phone call with your doctor.
5. CHECK OUT BY PHONE – No need to stop by the front desk in most cases. The follow-up visit will be scheduled over the phone immediately after your appointment.
In light of the COVID-19 virus; Evergreen Eye Center is asking patients to please adhere to the following guidelines to reduce exposure to yourself and others:
1. If you are able, please come into the building ALONE. If you do need assistance, please only bring ONE adult with you.
2. After we check your temperature, we may ask you to wait in your car until our staff is ready to see you.
3. If you are experiencing cough, fever, shortness of breath, or have traveled outside of the U.S. within the past two weeks, please call 800-340-3595 to reschedule your appointment.

Due to recent concerns regarding the health and safety of our community amid the COVID-19 (Coronavirus) outbreak, we want to assure you that everyone at Evergreen Eye Center is doing everything we possibly can to minimize the spread and to keep our patients and staff safe. Our clinics have always been cleaned on a regular basis and we have added additional measures to keep our facility sanitized and disinfected. Please be aware that these practices include monitoring the staff and patients that enter any of our five clinics. Coronavirus is a rapidly evolving global health issue that is affecting geographic areas differently; we are doing our due diligence to stay educated and informed. If you are sick or experiencing symptoms of upper respiratory infection and or fever please cancel and reschedule your appointment and stay home to recover. While we know potential cancellations and disruptions are not ideal, the health and well-being of our patients and staff must be our top priority. At this point, Evergreen Eye Center will remain open and will keep you informed should that decision change.
