Take fat out? Put fat back in?
It’s not surprising that this can be confusing. As you can see, the #undereyebags fat is thick & clumpy where as the fat used to re-volumize is smooth and gel-like (like filler). The goal is to remove the bags and create a smooth transition from the eyelids to the cheeks that will look natural and be long-lasting. Does that make sense?
It’s a very reasonable question: “how come you take out fat but then replace it with other fat”? You can see that the #undereyebags fat that we removed is clumpy. The fat that is used to re-volumize the eyelids and cheeks is smooth and gel-like, more like a #filler The goal is to create a smooth eyelid that seamlessly transitions into the cheek. Does that make sense?

What do you think we did?
It’s a very valid question to ask a surgeon “how often do you do this procedure?” My answer when it comes to eyelid surgery is 7-10 per week. The surgery maybe just upper or just lower eyelids, or both. Do you think that is a lot, or a little?

Other procedures tighten the skin but are not as effective treating the submental fat component because they are applied externally and may not penetrate deep enough.

What is Sculptra?
• #sculptra is an active dermal filler. This means it works by stimulating your own collagen production.
• It works gradually, so the changes look very natural
• Treatment is done every 2-3 months until goals are achieved
• Amount needed depends on the extent of volume loss, response to treatment and desired results. This patient had a total of 4 vials between 2018 and 2020. Treatment can be combined with traditional HA fillers such as #Restylane or #Juvederm for specific areas (i.e. lips, undereye).
• After the goal is achieved, treatments are done less frequently (every 9-12 months), with smaller amounts (1-2 vials).

Surgery is a big decision and can seem scary but it’s best to choose the right treatment from the start. I am an advocate of undereye filler IN THE RIGHT CANDIDATE!
Most undereye fillers look great immediately after treatment, so be cognizant of that when looking at the before/after photos. Problems may arise days-weeks after, if patient selection wasn’t ideal.

You sometimes have to take a step back to go forward. I dissolved some misplaced filler above and inside the lip, then re-did it. Do you think it’s better?

Don’t! Because I didn’t do anything. These pictures were taken 5 seconds apart.
In the top “before” picture, my hollowness looks worse because I’m looking up & wrinkles look worse because I’m smiling.
In the bottom “after”, everything looks so much better & smoother because of the position and lighting.
When looking at before & after pics on IG, be cognizant of the lighting, flash, shadows and position. If they are too different between the 2 pictures, take that into consideration of the result.
IPL photofacial is a great way to improve sun spots, pigmentation and unwanted freckles. This is after 1 treatment but a couple maybe needed to further improve the results. Of course, ☀️ block is necessary throughout the year.

What are these numbers?
▫️30 minutes (30-40) is the operating time to perform an upper eyelid lift on both sides
▫️48 hours of daytime icing (20 minutes every hour)
▫️6 days until stitches are removed

▫️3-6 months to see final results although most patients feel that they look good as early as 2-3 weeks after, depending on how quickly the swelling resolves

Many of you guys thought that this patient had fillers to correct the mild #undereyebags and #darkcircles 😉 but this is a surgical result of lower blepharoplasty with fat transposition. What the surgery allowed us to do, which fillers couldn’t, is to permanently remove the bags, re-utilize the fat and tighten the skin.

Eyelids can get so droopy that they obstruct vision. Then, surgery is performed to improve the quality of life, as well as improve appearance.

This is the result of the surgery shown in the last post (lower skin pinch blepharoplasty with canthopexy) + upper blepharoplasty. This was done to tighten the lower eyelid skin and improve the wrinkles. Photos taken a few weeks after surgery. Surgery was performed with local anesthesia and an oral sedative. We will possibly add a little filler to further refresh the eyes now that the skin tone is improved.
I am honored and grateful to be able to get vaccinated against COVID-19. I base my decision on my knowledge about the vaccine and the virus. I strongly believe that the benefits (personal & public health) highly outweigh the small risks that exist with any vaccine.
Who should get undereye filler?
Someone with dark circles & hollowness AND
1️⃣ None or minimal undereye bags
2️⃣ Excellent skin tone
I am VERY selective in who I treat with filler in the eye area. Trust me, all the trouble of dissolving it and then doing what should have been done in the first place... 🤦🏻♀️, it pains me to see patients go through that.
Moral: make sure you are a good candidate for whatever procedure you are having done, and that your provider is honest about it!

Platysmal bands are those vertical lines on the neck that are often seen as the neck ages, especially in those with thin skin. Treatment options include 1️⃣ Surgery 2️⃣ Botox 3️⃣ using the #ellevate suture to cut them, which is what this patient had. It’s a minimally invasive procedure done in the office with local anesthetic and takes 20 minutes to perform. It can be combined with other treatments to rejuvenate the neck. The nice advantage over Botox is that the results last years, not months.

My lovely patient wanted an improved neck and jawline but wasn’t ready for surgery. #facetite + gentle liposuction (lipotite) was therefore her best alternative. These are just 1 month results and the best is yet to come - continued improvement for the next 6-8 months. Results last 5-7 years.
My patient had #ptosis - his lids were droopy and sitting too low on the eye, making him look sleepy and affecting vision. I tightened the eyelid muscle to lift the eyelid. He did not wish to pursue a blepharoplasty (to remove eyelid skin) as he wanted to retain the masculine eyelid fullness and look natural for his age. Every surgery should be customized to that patient’s goals, not one fits all! Agree?
