1. If you have a cough or fever we ask that you call to reschedule your appointment.
2. If you have traveled anywhere outside the state within the last 30 days, please call to reschedule your appointment.
3. Please wear a mask to your appointment.
4. Please do not have anyone accompany you to your appointment. Unless you are a parent of a child or an interpreter, we ask that you enter the building alone.
5. Please practice social distancing while waiting to be seen and while registering at the front desk.
