Hashimoto’s and Inner Ear Issues
Hey, people! This week I’d like to explore issues involving Hashimoto’s, hypothyroidism and inner ear issues.
This came up in a number of questions when I asked for topics and it is a potential complication of Hashimoto’s that you should be aware of. Like thyroid eye disease it is also something that doesn’t seem to get enough attention, in my opinion.
Unfortunately, I have experienced both. The blessing and curse of having Hashimoto’s and working with people to help them understand and heal from it. I can often speak from experience (good and bad).
First I’ll provide some background, then I’ll explore some options about things you can do about it.
A Little Background
Thyroid hormone is essential for the physiological development of many tissues in the body and the cochlea is no exception.
Hypothyroidism and dysfunction of the thyroid gland can result in problems involving the auditory system. This is true in the developing fetus and it is also true in adults.
Many studies have shown an association between hypothyroidism and hearing loss. Tinnitus and vertigo have also been linked to these auditory disorders.
Patients with hypothyroidism are at a higher risk of developing tinnitus and hearing loss itself is the most common cause of tinnitus. People with autoimmune thyroid disease have a higher risk of developing tinnitus.
Studies on the effect of hypothyroidism on the ear have shown that the inner ear can be impacted. Especially the cochlea.
Particularly structures known as the tectorial membrane. It can also impact inner and outer ear hair growth and cause debris and fluid to build up in the cochlear duct.
Hypothyroidism can lead to both neurological and morphological (physical) defects in the development of part of the inner ear called the organ of Corti. This is kind of like the ear’s microphone. Researchers believe this suggests that thyroid hormone exerts direct effects on the cochlea.
People with Hashimoto’s are at higher risk of developing hearing problems than healthy individuals. This can be secondary to Hashimoto’s and it can also be found as another autoimmune disorder known as autoimmune inner ear disease (AIED).
This should be considered if you have a history of Hashimoto’s and you develop an acute onset of symptoms such as tinnitus, a feeling that your ears are blocked and you can’t unblock them, excess fluid in your ears, vertigo, etc.
As with many problems involving autoimmunity, inflammation is at the root of this problem and it may respond to treatment with steroids.
AIED causes inflammation of the inner ear structures. The most common symptom is fluctuating/progressive sensorineural (inner ear) hearing loss with up to 50% of patients also experiencing tinnitus and vertigo.
Studies have found that hearing loss was determined to be sensorineural based (found i the inner ear) and MRI findings showed cochlear inflammation. There is no definitive test for AIED. Elevated ESR, CRP and a positive ANA or RA factor may be consistent with the diagnosis.
Some other autoantibodies have also been associated with this disease including:
Anti-HSP70 (Heat shock protein 70), Anti-cochlin, Anti-beta actin, Anti-CTL2 (Choline transporter-like protein 2), Anti-collagen type 2
But their diagnostic and prognostic role remains controversial. MRIs can help identify the location
and rule out other causes like acoustic neuroma and ototoxic neuropathy.
Some other autoimmune disease associated with AIED include:
• Cogan’ syndrome
• Systemic lupus erythematosus
• Vasculitis
• Rheumatoid arthritis
• Systemic sclerosis
• Relapsing polychondritis
• Polymyositis-dermatomyositis
• Hashimoto’s thyroiditis
• Ankylosing spondylitis
• Behḉet disease
• Inflammatory bowel disease

“I have several patients who work at CMC, ASH, Trader Joes or Miners. They just can’t hear clearly through the plexi-glass and masks.” For essential workers struggling to understand others, Hearing Aid Specialists will add this new program at no charge.
Setting up the new mask-mode takes about thirty minutes and it can be programmed to 95% of all brands of hearing aids. It is done at his office by appointment. Our schedule is filling up quickly. We have two offices, one in Atascadero and one in San Luis Obispo. If you have questions or need more information, please call 805-460-7385.
