Veteran’s Pinning Ceremony brings closure and gratitude to our patients and their families by honoring their service to our Country. Hospice of San Joaquin patient, Wilbert Henneberg, was honored at his home and was joined by his family and friends, and Hospice of San Joaquin staff who share words of invocation, past stories, and a thank you for all that he has done.
Hospice of San Joaquin offers monthly zoom bereavement support groups on an ongoing basis. These groups are for individuals who have experienced the loss of a loved one. Support Groups provides essential tools to move through your grief journey including connecting with others experiencing similar grief pain. November marks 40 years of care for Hospice of San Joaquin. Today, we hosted our 40th Year Anniversary Drive-Thru Celebration, thanking staff, board members, volunteers, and Butterfly Auxiliary for their endless commitment in providing compassionate end of life care for patients and their families.

National Hospice & Palliative Care Month is more than recognizing hospices across the country for the work they are doing, it's also about highlighting the high-quality, compassionate end of life care our skilled professionals provide.
Our “team” consists of the patient’s Primary Care Physician, the hospice Medical Director, RN Case Managers, Licensed Vocational Nurses, Hospice Home Health Aides, Social Workers, Chaplains, and trained Volunteers. Through this interdisciplinary approach, we will:
✔️ Provide an informational visit with a nurse prior to admission
✔️ Manage the patient’s pain and symptoms
✔️ Provide necessary comfort medications, medical supplies, and equipment
✔️ Assist the patient and family with the emotional, psychosocial and spiritual aspects of dying
✔️ Provide education for family and significant others on how to provide care for the patient
✔️ Provide bereavement care and counseling to surviving family and friends

Volunteers are an integral part of our hospice team. Veteran volunteers are paired with a patient who is a veteran or has a military background. Similar backgrounds and shared experiences allow for deeper connections between the patient and volunteer. The Veteran to Veteran Program is a great way for our veteran volunteers to connect with their veteran patients.

Our Memory Bear Volunteer, Donna, brought in hand-quilted Memory Bears for patients.
Memory bears are made from the clothing of loved ones and are treasured as a huggable keepsake. Hospice of San Joaquin patient families are eligible to receive a complimentary bear. Additional bears and ones available to the community are available for a $30 donation per bear.

Pacific Palliative care is a home-based, outreach, consultant support service for the discomfort, symptoms, and stress of serious illness. Supporting our Community -
We know the process of loss doesn’t end when a loved one passes away. We offer many programs throughout the county for not only our patient families, but for anyone who finds themselves working through loss.
In 1985 we expanded our focus to those who are left behind with adult and children’s bereavement groups and a school grief program. These programs have continued to grow and today 80% of participants come from the community and the remainder are our patient families.

Hospice care is the model for quality, compassionate care for people facing a life-limiting illness or injury. Hospice care uses a team-oriented approach to expert care —called an interdisciplinary team (IDT) approach. The IDT team addresses the physical, psychological, social, and spiritual “parts” which compose us all; and addresses those issues causing patients and their families’ pain. Hospice of San Joaquin welcomes, Evia Briggs Moore, as new Member of the Board of Directors for Hospice of San Joaquin. Moore brings in over 50 years of experience as an American Librarian, American Missionary Association Fellow of New York City and University of Wisconsin Fellow. Born and raised in Mississippi, she earned her Bachelor of Arts at Tougaloo College, Master of Library Science at Syracuse University, and finishing her Postgraduate at University of Wisconsin in 1973. Her career stems from working in public schools as Elementary School Teacher, Acquisitions Librarian at Tougaloo College, Assistant Professor at Jackson State University, Librarian Consultant Prentiss at Mississippi Institute, before residing at her current position as Librarian at San Joaquin Delta College. She has held positions as Chairman of the Youth Concert Stockton Symphony League from 1987 – 1988, Member Scholarship Committee Stockton Chapter Links Incorporated since 1980, Member of California Library Association, Delta Kappa Gamma, Phi Delta Kappa, Delta Sigma Theta (Stockton President Chapter from 1984 – 1986), and American Association of University Women, American Library Association. Her accomplishments have earned her as a listed noteworthy librarian by Marquis Who’s Who. On her free time, she enjoys reading and gourmet cooking, and spending time with her husband, Jones Ambrose Moore and her son Robert Vance Moore.
We are honored and excited to have her serve as new Member of the Board of Directors for Hospice of San Joaquin. Her dedication and background in the education system will further help continue to foster our commitment of providing compassionate and high-quality end of life care for the patients and families we serve.

The American Cancer Society’s annual Cancer Statistics reports improved survival overall for those facing cancer. The improvement is most pronounced in lung cancer, thanks to improvements in treatment of non-small cell cancer. We have come a long way since the 50’s and 60’s when the word cancer was said in hushed tones due to the likelihood of it being a terminal illness.

Hospice of San Joaquin’s Tracy Butterfly Auxiliary invites you to our Annual Eggstravaganza, Saturday, March 27, 2021 from 11:00 a.m. to 1:30 p.m. at the Masonic Lodge located at 1601 N Tracy Blvd, Tracy, California. A twist on the traditional Easter egg hunt, guests are invited to a vendor-style Eggstravaganza event with the theme "Hop Around for Hospice" where participants can walk to stations to purchase eggs, pick up their lunch, and pick up their prize basket at the end of the event. Each egg offers a winning number for one of the hundreds of baskets. The excitement is accompanied by a pick-up or drive-thru lunch. This is a great event for the whole family.

Hospice of San Joaquin welcomes Dr. Goswami as one of our new Team Physicians! 👇
Dr. Goswami joins us with expertise in Internal Medicine, Pulmonary Diseases, and Critical Care. He attended Government Medical College of Amritsar, India where he received his Bachelor of Medicine & Bachelor of Surgery (M.B.B.S.) and earned his Pre Medical Degree at Government College, Hoshiapur, India. Dr. Goswami then completed his Internship & Residency in Internal Medicine at St. Elizabeth Medical Center, his Fellowship in Critical Care Medicine at Brown University, and Fellowship in Pulmonary Diseases & Critical Care Medicine.

Hospice of San Joaquin is excited to welcome Dr. Puthillath as new Pacific Palliative Care Physician!
Dr. Puthillath is the former Chief of Staff and is currently an active member of the Community Board at Lodi Memorial Hospital where he also serves as the Chairman of the Bioethics committee. He is the Co-Chair on the Breast Cancer Center of Excellence at St. Joseph’s Medical Center in Stockton. Dr. Puthillath completed his medical training from Mysore University, India. He then completed his internship and residency training in Internal Medicine at McLaren Regional Medical Center, Michigan. During his residency, he was awarded the PGY-2 for outstanding resident of the year and the “Ambassador Award” for the best outgoing resident. Dr. Puthillath completed his fellowship in Hematology and Oncology at Roswell Park Cancer Institute (RPCI), New York. Dr. Puthillath has numerous research publications in peer reviewed oncology journals and has actively presented in the American Society of Clinical Oncology meetings.
