A facial fracture is a potentially devastating injury that affects all ages. This patient had a fracture of the cheek bone and floor of the orbit supporting the globe of the eye. Fortunately, we were able to operate in a timely fashion to restore her bony support. At the same time, we were able to avoid some of the problems that can happen with the soft tissue such as lower eyelid retraction or unsightly scarring. Our office remains available to consult for facial fracture repair immediately after injury, and also for reconstruction months or years after the original injury. After healing, sometimes the bone needs to be repositioned or augmented. Other times camouflage may be possible with non-surgical injections, lasers, or scar revision in the office. Here is another ear molding patient who just finished 4 weeks of treatment. This child has a third branch, or crus, in the shape of the cartilage. In the first few weeks after birth, we are able to mold the cartilage to correct this extra fold. If not treated now, surgery would be needed, and almost always with inferior results. Here you can see some of the minor effects that may happen. In the first photo, there is a small scab where the pressure was on the cartilage. This usually heals in a few days without any lasting effect, though if early on, would require a pause or earlier end in treatment to allow it to heal. The second photo shows some of the dry skin that can build up while the ear is cocooned. Please call our office at 972-331-1900 for an appointment or for more information. Ear molding is a way to gently reshape an infant's ears while the cartilage is pliable. A family told me it reminded them of having braces. One difference from braces is that a course of ear molding is only 2-6 weeks. Another difference is that treatment is best started in the first few weeks of life. In those early weeks, maternal hormones are still circulating in the baby, but are gradually decreasing. The sooner treatment is begun, the better the result, and fewer visits are needed. Because this is time-sensitive -- and does not use valuable hospital supplies -- I am still performing this procedure in the office when indicated. We continue to take all possible precautions for the safety of our families and staff. I am also available at some hospitals in the region to begin treatment while still in the hospital. Beginning therapy in the hospital is helpful for children requiring a longer admission after birth (including for premature babies). As always, safety comes first. We have been busy moving to virtual evaluations and follow-ups wherever possible. Emergencies and time-sensitive conditions still need to be seen in person. For these patients, we are spacing out appointments, avoiding time in a waiting room, and sanitizing surfaces and rooms immediately. Visitors are screened at the entrance to the building, with no additional visitors allowed. Masks are being worn. Ironically, my available hours may be expanded, as our elective surgeries are delayed. Please feel free to arrange for a virtual consultation. We have HIPAA-approved telemedicine we have used for years already, and can line up procedures for after this cloud has passed. Take care, everybody, in these uncertain times. The movie was surprisingly good, too, though different enough to recommend reading / seeing each one. It’s hard sometimes to put aside winning an argument, to instead slow down and listen. Breast reduction is a commonly performed procedure to help improve neck and back pain, deep bra strap grooves, and recurrent rashes. Over 100k women (and 20k men) undergo this surgery annually in the USA. 2-5 pounds or more may be removed, immediately improving many of the symptoms. Insurance plans will sometimes cover the surgery, though individual plans vary, and it is necessary to have it authorized ahead of time. A lift alone (mastopexy) falls on the same spectrum. The nipple and aerola are repositioned aesthetically with the lifted breast, but with much less volume removed (or sometimes with volume added with liposuctioned fat or an implant, an augmentation-mastopexy).
