Children’s Health now offers a service specifically dedicated to caring for the pediatric population with complex, chronic medical conditions. For primary care physicians in the community, time and resources may not be available. Children’s Health is here to help. Complex Care Medical Services is comprised of two main programs - Care Coordination Program and Medical Home.
