To our valued JRFM patients,
Jordan River Family Medicine is excited to offer the option of Telemedicine visits for our established office patients. To be eligible to use the telemedicine format you must have an established relationship with the physician and have been evaluated within the office in the past two years.
Certain types of visits including follow up visits for medication, labs, chronic disease management (diabetes, high blood pressure, ADHD, mood issues, etc), weight loss counseling, initial acute illness evaluation (acute sickness, rash, back pain, etc) may be appropriate for telemedicine visits.
By no means will telemedicine visits replace our in-office visits but for now they can be a helpful, safe way to stay connected. We will be using the Zoom Video Conference format so any patient using his/her smart phone or Ipad will need to download the free Zoom Video app prior to the visit. If using a desktop or laptop computer then you do not need to download the Zoom app and will just connect to the conference through an email link sent by the physician at the time of the appointment.
We will be billing insurance for the medical visits like usual. The front desk will collect copays and get appropriate personal information updated prior to the telemedicine visit. All patients using telemedicine need to have an updated phone number, email address, insurance card, signed financial agreement and active patient portal log-in prior to visit check-in. Patients who need to provide updated personal information can send us information through a private patient portal message prior to scheduling an appointment thus saving time.
While we work out the kinks please be patient with the staff and know we are all trying to provide safe, effective care for you and your family. Thank you for being part of our Jordan River Family Medicine Family.
Sincerely,
The Board Certified Family Physicians and Staff of JRFM
Some of you are asking what you can do to help. It feels so good to know we live in such a supportive community.
We know many of you have supplies stored. In no way do we ask you to do something that you feel would put you at risk. However, if you have any hand sanitizer, Lysol , bleach wipes or N-95 masks, we ask that you consider donating some to help with the fight.
Stay safe everyone and know we are here for you.
We want to reassure you that we are taking the COVID-19 Pandemic seriously. Starting Monday, March 16, 2020 we are opening an isolation area for any patient who has suspicipus respiratory symptoms with fever, cough, shortness of breath, etc. (Our goal is to allow healthy patients to stay healthy and isolate those who are sick.)
- If you need to be evaluated for a respiratory illness you will be scheduled into the Respiratory Clinic being operated from 2-5 PM each day.
- You are not allowed to enter the main building and will be asked to stay in your car until your appointment.
- During your urgent care visit you are not allowed to have any other person in the isolation room with you with the exception being one parent escorting a sick child.
We are here to provide safe, effective care and want to help stop the virus from spreading. We appreciate your cooperation in this tough time.
IF YOU ARE HEALTHY AND CAN AVOID AN OFFICE VISIT AT THIS TIME IT IS RECOMMENDED!
*We will be closed for the Thanksgiving holiday beginning Thursday November 26th through Sunday November 29th. We will return to the office on Monday November 30th. Please plan your health care needs accordingly.*
