Our management team at Logan Elm Health Care Center, with nearly 250 years of combined experience offers a 101 bed skilled & intermediate unit that is certified for both Medicare & Medicaid. This is important in case your loved one’s health or financial status changes because he or she can continue to receive the needed services without having to move to another hallway located in the facility, or without moving to another facility. When working with elderly, it is crucial to keep change to a minimum.

Our admission coordinator, business office manager or administrator look forward to answering any questions you may have regarding finances. We accept several payer sources including private pay, long term care insurance, Veteran administration, Medicare, Medicaid, Medigold and many Managed care insurances.

Testing has been canceled as positivity rate has improved in Pickaway County.
TO ALL EMPLOYEES.....CMS is now requiring all Pickaway County Facilities to COVID test employees 2 times a week until positivity rate decreases in this county. Subsequently we have scheduled testing for (TOMORROW) FRIDAY SEPT 4th from 7a thru 10 a. We will proceed with testing on the 8th as well. We have been so thankful to each and everyone of you for maintaining a safe environment to those we care for. Your dedication and commitment is very much appreciated!!! THANK YOU TO OUR AMAZING STAFF!!!!
Update: fortunately, and thanks to God and all the prayers, COVID testing from Friday continue to all be negative! Some isolated testing results are still pending but we continue to prayerfully have faith that they too will be negative! Following set precaution mandates, testing of staff and residents will ocurr again tomorrow with results anticipated by weeks end. Unfortunately, in our continued efforts to ensure the safest environment for those living here, all outdoor visits have temporarily been suspended until we receive all results from our second round of testing. We thank you all for your support and continued prayers!
We would like to share some news and provide clarification. Today we were informed that an employee tested positive for COVID. In an abundance of caution and per guidelines set forth by the Ohio Department of Health we immediately began testing of employees and residents. Testing will once again be repeated on Tuesday. Fortunately, preliminary testing has thus far shown negative results. However, we are asking all our friends and community for prayers. We have worked diligently to ensure the safety of our most precious guests and residents. And we will continue to do so. Thank you all and thank you for continued prayers!

To our Community here is an opportunity to get a free COVID test!
A special note to the families of those we care for....Visitation has resumed. While we are delighted to once again start visitation, our concern for safety remains very high. As cases of COVID rise in our community, the risk grows greater to our residents. Subsequently, we have tightened our guidelines as suggested by CMS. School aged children will not be permitted to visit. The facility is also highly encouraging visitors to be COVID tested and have negative results no more than 2 days prior to visit. Rapid testing may be performed by the facility prior to each visit for those who do not provide documentation of negative COVID results. This test takes 15 minutes. Each visit must be scheduled and will be limited to 30 minutes. Inside visits will take place in the dining room and limited to 2 families at the alotted time. Parking for visitors will be designated in the back of the building with entry through side doors opposite the facility dining room. There will be a limit of 2 guests and each guest must have their temperature checked and wear a face mask during their entire visit. Social distancing must remain in effect. Failure to comply with safety standards will result in exit from the facility. We sincerely hope you understand the measures we are taking to ensure continued safety of your loved one. We pray daily for this pandemic to end!

COVID update: Our goal is and will remain to be transparent with our residents, their families, our staff and our community. Fortunately all testing from tonight have shown negative results. We still have some testing to do and results to wait for, but rapid results have consistently been negative. Thank the Lord! Per governmental mandate and guidelines we will have to continue testing every 3 to 7 days for a period no shorter than 14 days. As a result, we will have to temporarily suspend in house visitation. Of course, window visits and virtual visits will continue. We understand the challenge and difficulty this is for our residents families. Please know that we sympathize and are doing our best to not only ensure safety but that psychosocial needs are being met as well. We understand your frustrations. We understand your anger. But we will continue to do what we have always done....care for your loved one in the best way we know how to do. Please continue to pray for the facility and for the continued safety of those who live within its walls.

Asking our community to continue prayers as our industry faces increasing risk of COVID. As our county and state show a rise in covid positive cases, we have seen 2 other local nursing homes have our counties first resident positive cases. Logan Elm Health Care remains vigilant and aggressive with infection control measures and our own staff take extra precaution when not at work. But we still ask for prayers. Our Services are growing! We are proud and excited to begin training for a new Vent unit, offering our community a level of service that it currently does not have. A dedicated team of staff will oversee the Ventilator Unit. Looking forward to opening the unit in early 2021!

REMINDER TO STAFF.....Tomorrow(Wednesday the 16th) is our next scheduled COVID testing date. This is a PCR test. Testing will be from 7a to 10 a. Night shift staff scheduled to work that evening can take their test then. All other employees are required to be tested between the hours of 7a to 10a. Thank you to an amazing staff!!!!!

To all of our families we respectfully share the following change in procedures. All incoming calls regarding patient updates will be logged by the individuals handling phone calls. The log will then be shared with the nurse on the hall and the nurse will call back and provide those updates. We appreciate your understanding with this practice. Our nursing staff are working hard to ensure the best quality of care is being delivered and have found it difficult at times to step away from care for each call that is coming in. Once they have a moment, the call will be returned. Again, we appreciate your understanding. Thank You!
COVID VACCINATION UPDATE: To all our staff and Resident Representatives the vaccinations for COVID begins January 9th with the second date on January 30th. A third date has not yet been scheduled. A facility representative will be reaching out to all families to gain consent or decline of the vaccination. All staff are encouraged to take the vaccine and must either complete a consent or a denial form. Staff must also make the facility aware if they are wanting the vaccine so we can be assured in having enough brought to us. This has been a difficult and challenging season for everyone. We sincerely appreciate all the prayers and support from our community!
