Last week’s case, Le Fort 1 advancement, bilateral sagittal split osteotomy and genioplasty. This patient presented with dentofacial asymmetries and obstructive sleep apnea. Working closely with our craniofacial orthodontist, we used virtual surgical planning to plan the surgery and create models before entering the operating room. Plastic surgeons and maxillofacial surgeons are able to use computer engineering and 3D printing technology to pre-plan the entire surgery in order to save time, reduce cost, and improve surgical precision.
In this surgery, I performed an osteotomy or cut the maxilla from the face and performed an osteotomy or cut the jaw and chin. I then advanced the bones and secured them into their pre-planned positions using 3-D printed guides, splints and titanium plates. The goal of this surgery is to improve the dentofacial asymmetries and improve the airway by opening it up.

In some cases, plastic surgeons can use donated human rib cartilage. The video demonstrates how the cadaver rib looks after it has been prepared for implantation. Cleft lips can occur centrally or laterally at the corner of the mouth. A lateral cleft lip is also called a Tessier 7 cleft and results in macrostomia or a large mouth. Plastic surgeons with additional training in craniofacial surgery treat many different types of facial clefts. In this case, the right corner of the mouth was reconstructed to rebuild all layers of the mouth, the mucosa, muscle, and skin. The patient only wanted a mild amount of augmentation. From the post-operative photos, you can see the waist is narrower, the hips are wider, and the buttock more defined, projected, and lifted. This patient is 3 months out from 360 degree liposuction of the abdomen, liposuction of the bilateral thighs, and fat transfer to the bilateral hips. The patient did not want a BBL or augmentation of the buttock with fat transfer. On frontal view, you can see improvement in the waist and hip contour. On profile view, the waist is significantly reduced and even without augmentation of the buttock, you can see improvement of the buttock projection with just liposuction of the lower back.
