- Ask your physician for a referral to Porter Hills Home Care. If you are hospitalized, you can request Porter Hills Home Care from the discharge planning staff.
- After we receive the order, our staff will contact you within 24 hours.
- A nurse or therapist will perform a physical assessment, review your medications and answer any questions you may have about your home medical care. Together, we will create an individualized plan to achieve your personal health goals.
