To help expedite the admission process, our staff will take some information over the phone such as demographics, health insurance information, and method of payment. We will verify insurance benefits prior to your initial visit to the clinic. In order to serve you better, we have included a link on this page to the admission documents. After completing them in the comfort of your own home you can bring them in at the time of your first visit. We do file directly to your insurance carrier, however the patient is responsible for their co-payment, deductible, or non-covered amounts at the time of service.
