About

I have over 6 years of experience working for the San Antonio Spurs team physicians. I specialize in treating athletes and have extensive experience with middle school, high school and college athletes. Athletes require different care due to the physical demands on their bodies. I design specific programs to address these demands. Most physical therapists have a broad knowledge of orthopedics and spend most of their time treating a much older population. They have limited time, resources, and knowledge in working with athletes. At Puckett Physical Therapy, we are located inside an athletic training center equipped with all the tools needed to help you succeed as an athlete.

Rehab beyond the first 3 months after an ACL injury is a MUST for anyone that wants to do more than just walk... —— Have you been told “just go to the gym on your own now” or “go lift weights” or “just go practice with the team now”? How can we improve engagement with our ACLR patients to continue their rehabilitation beyond 6 months to at least 9 months post-op, where we know there is a less chance of 2nd knee reinjury? For me, I've seen great engagement and extension of rehab time via with small group exercise sessions. Clinicians, how about you? Shared from a colleague of mine and I agree wholeheartedly. He also added in his comments that athlete confidence is a factor as well. I tell them I never clear anyone before 9 months and articles like this are exactly why. There is simply too much to work be done and clearing someone at 6 months is way too early. A patient must regain strength and confidence before being cleared and this takes time with a lot of hard work. 🔔Time is ONE factor of the return to sport process. Strength and psychological testing must be properly done as well. 🤔My motto is, let’s rehab the right way the first time. I’d be weary of any ortho, PT, ATC, etc, that tells anyone they’ll be cleared in 6 months. This is simply not ok and we as rehab professionals NEED to do better. 🤯Pictures/content below are from @mickhughes.physio 📖 Another study highlighting that non-professional athletes/patients should not rush back to sport earlier than 9 months post-op ACLR. 🏋️Building on the work by Grindem (2016) who showed a 51% increased risk of knee re-injury (not just ACL injury) for every month an athlete returned to sport earlier than 9 months; this paper by Beischer et al (2020) showed that those athletes who returned to "knee strenuous sport" earlier than 9 months (Tegner activity >6), had a 3-7x greater risk of a 2nd ACL injury compared to those who delayed return to sport til after 9 months. 👉𝗔𝗿𝗲 𝘆𝗼𝘂 𝗶𝗻𝘁𝗲𝗿𝗲𝘀𝘁𝗲𝗱 𝗶𝗻 𝗼𝗻𝗹𝗶𝗻𝗲 𝗔𝗖𝗟 𝗿𝗲𝗵𝗮𝗯/𝘁𝗿𝗮𝗶𝗻𝗶𝗻𝗴 𝘄𝗶𝘁𝗵 𝗺𝗲? Playing back video from one of my baseball ⚾️ players getting back in to throwing. —— During the rehab process it’s important to see your athlete performing the sport specific requirements that their sport demands. You can learn a lot by watching them! —— This is why true sports rehab cannot occur within the confines of a traditional healthcare clinic. —— It is also our job as sports PTs to make sure the athlete can perform and withstand the demands of their sport before sending them back to their sport. That means a baseball ⚾️ player must throw before we tell them to go throw in practice! —— The other HUGE benefit here is building the athletes CONFIDENCE and eliminating fear and hesitation of reinjury by giving them quality graded exposure to the movement that they were previously injured with. —— Athletes deserve true sports rehab. In the late stages of rehab an athlete needs drills that prepare them for their sport. For an ankle sprain, ankle band exercises won’t cut it. Here is one of many exercises we are using to get @cruise.lafreeda ready for his return to basketball 🏀!!!
Do you have full knee extension❓ Is it equal between your two knees❓ . Restoring full knee extension is absolutely crucial after any knee injury or surgery, especially an ACL. The longer it takes to get it back, the harder it is to get full motion restored. Also, your quad cannot perform at its best if your knee doesn’t move in to full extension. Quad strength and performance will be greatly limited by lack of terminal knee extension. So do this: . 📌Sit with your legs straight in front of you 📌Squeeze your thighs trying to lift your heels off the ground BUT keeping the back of your knee pressed ⬇️ into the floor. . 🔎Do your heels lift up? ✅ 🔎Is it equal between right and left?✅ . If the answer is no, then you need to work on your knee extension! . So what do you 👀 in the video? Good knee extension? Equal between right and left? Comment below!
