Bring them to your first visit or even fax them.The Patient Information Form is required, while the second two are optional depending on your individual situation. Make sure you have Adobe Acrobat installed on your computer so you can print the forms easily. When you arrive for your first appointment, bring your patient forms to the receptionist already filled out. Next, you will have a consultation with Dr. Steven Rosa to discuss your problems and possible treatment plans. At this first visit.
