Lifespace Communities would like to introduce the opportunity to become a Registered Nurse Lead at The Stayton at Museum Way, a CCRC community located in Fort Worth, TX.
The Registered Nurse Lead is responsible for supervising Assisted Living & Memory Support services and maintaining quality and adequacy of nursing practice. This role is responsible for demonstrating a commitment to customer service through resident focused initiatives that support the mission of the community and Lifespace Communities.
A few other details about the role:
Assist the director of Assisted Living and Memory Support with all functions of the AL/MS departments and assumes responsibility when the director of nursing services is unavailable.
Oversight, monitor, and initiate physician orders, verify all orders received are transcribed accurately in electronic records and treatment plan, administer medications and treatments are provided according to orders.
Establish protocol to ensure care plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
Monitor and counsel with nursing staff on the administration of treatments, proper documentation of status and observation reactions to medications and treatments.
Direct team members and create “at risk” assessments. Orchestrate investigations, corrects and follows up on resident and family concerns as needed in a timely manner.
Administer essential nursing functions including starting intravenous fluids, prepare, administration and charting of all medications.
Facilitate communication with families regarding change in medications and/or changes in the resident.
According to standard operating procedures, ensure correct staffing levels. Develop, direct and monitor nursing and nursing assistant assignments adjusting based on census and level of care required.
Assist with employment related activities to include team member safety, hiring, performance management, corrective action, team member engagement and recognition, and training.
Provide an accurate and thorough report to the on-coming shift in order to ensure the continuity of maximum resident care.
Assist with cost control programs
Direct team members in emergency situations and contact appropriate parties as needed.
Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
Obtain knowledge and demonstrate the principles of Living Lifespace, person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, team work and quality. Team member will support, actively participate and act in accordance with the principles of the Living Lifespace service culture and be familiar with and fluent in the language and service pillars of Living Lifespace.
Maintain regular and punctual attendance at work and meetings.
And here’s what you need to apply:
Nursing diploma or associate’s degree in nursing from an accredited nursing program is required. A Bachelor's degree is preferred.
Two years of Nursing experience in a long-term care facility preferred.
One year supervisory/leadership experience with demonstrated progressive responsibilities preferably in a long term care facility.
Or equivalent combination of education and experience.

Registered Nurse Lead
Lifespace Communities would like to introduce the opportunity to become a Registered Nurse at The Stayton at Museum Way, a CCRC community located in Fort Worth, Texas. This position will support a 46-bed health center. The shift that we are currently seeking is (Double weekends 6a to 6p; Mon-Fri 2p to 10p and Sat & Sun 10p to 6a).
The Registered Nurse is responsible for collaborating with physicians and an interdisciplinary team to provide nursing services to resident. This position also provides support to family members as well as supervising assigned team members. This role is responsible for demonstrating a commitment to customer service through resident focused initiatives that support the mission of the community and Lifespace Communities.
A few other details about the role:
Participate with members of the interdisciplinary team to review, plan, coordinate and evaluate resident’s care.
Documents the resident’s condition and nursing needs accurately and in a timely manner. Reports pertinent observations and reactions regarding residents in a timely manner.
Oversee and provide leadership and discipline to licensed practical nurses and certified nursing aides. Develop, direct and monitor nursing assistant assignments adjusting based on census and level of care required.
Communicate staffing problems and needs to supervisor. Escalate team member performance issues to supervisor; participate in counseling and disciplinary action to the extent permitted by the State Nurse Practice Act.
Execute treatments as necessary while document status and observes reactions to medications and treatments.
Generate, prepare, administrate and chart all medications.
Initiate physician orders, verify all orders received are transcribed accurately in electronic records and treatment plan, administer medications and provide treatments according to orders.
Facilitate communication with families regarding change in medications and/or changes in the resident.
Establish protocol to ensure care plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
Coordinate admissions, discharges and transfers in order to deliver quality customer service.
Introduce an accurate and thorough report to the on-coming shift in order to ensure the continuity of maximum resident care.

Coordinate nursing services with other resident services to ensure the continuity of the resident’s total regime of care.
Oversee and assist in assessment, coordination, planning, monitoring, implementation and evaluation in case management and administer direct resident care.
Assist in developing policies; directly responsible for monitoring and enforcement of set policies.
As assigned participate in assigned resident care programs which may include the Interdisciplinary team, Resident Care Planning Team, the Restorative Care Program and other programs as necessary to provide optimal care.
Foster an environment which acknowledges and supports each resident and their rights to make choices regarding their surroundings and care.
Meeting federal and regulatory requirements ensure appropriate staffing levels and forecast team needs with fluctuation of workload.
Assist with employment-related activities to include team member safety, hiring, performance management, corrective action, team member engagement and recognition, and team member development and training.
Implement, monitor, and report on assigned cost control.
Participate and facilitate, as needed, resident, quality, regulatory and safety meetings specific to the department and community.
