Hello to our Orthodontic Family and Friends, Thank you for your care and patience as we have been temporarily closed in compliance with the CDC’s and Governor’s directives during the COVID-19 pandemic. We are looking forward to returning to orthodontic care starting on Tuesday, May 12th, but only with strict adherence to our newest safety measures for both patients and staff. We are taking the safety protocols very seriously, and with that in mind, we have a 10-point plan to maximize health and safety in our office going forward: 1) All patients over the age of 18 or legal guardians of minors will be required to fill out a consent form and will be asked a short series of screening questions before being seated. If you would like to print and fill it out beforehand, it will be posted on our website at https://orthodontistdallastx.com/AAOICSuppConsentCombined.pdf. We will also send a link to these forms on your appointment reminder emails, and it is also attached to this email. 2) ALL people entering our office must be wearing a mask, even our younger patients. We are only able to stock enough disposable masks for the use of our staff, so we ask that each person brings their own. A bandana or scarf covering the mouth and nose will also suffice. 3) All people entering our office will have their temperature taken (no-touch forehead thermometer) and will be provided with hand sanitizer to use. 4) Our reception area will be closed, but there will be a few standing "stations" marked that are 6-feet apart from each other if you would like to wait inside the office. Otherwise, please arrive at least 5 minutes early and call the office when you arrive at (214) 692-5688. We will call you when your treatment chair is ready. While you wait, please fill out the wellness survey and informed consent attached to your appointment reminder if you have not already completed it. 5) We will have a "no-touch" policy, meaning that our front door will remain open, our front desk team will sign you in verbally (no keyboard check-in), and no coffee area, video-game, magazines, tooth-brush station, or bathroom-use (except emergencies). 6) Only the patient is permitted in the office. We will make exceptions for the younger patients or those who need special assistance. In that case, one adult may accompany the patient – this person will also be asked to complete the wellness survey, wear a mask, and have their temperature taken. Please no siblings or other guests. Though our favorite part of our office is the family atmosphere, right now we have to love the whole family from afar. 7) We are going to be seeing fewer patients than usual in the clinic area at a time, and patients will be seated as far apart as possible, with a minimum of 6 feet. Dividers will be installed between chairs where necessary. Our tooth-brushing area will be closed to prevent cross-contamination, so please brush and floss well prior to the appointment. 8) This “socially distant scheduling” will unfortunately make finding the perfect appointment time a little more difficult. We understand many of our patients are concerned about not having been seen in our office for some time. We empathize 100%, and so we will be working very hard to catch up on over 700 missed appointments. We will have to see every patient in the order that they were originally scheduled, unless there is an urgent concern. We are committed to getting everyone back on track as soon as possible. 9) We have invested in multiple medical-grade air purification units (such Blue Air and Molekule), which also have UV/electromagnetically-enhanced HEPA filters to continually clear the air of pathogens throughout the office. 10) In addition to our already rigorous sterilization and disinfection program that has always exceeded CDC and OSHA guidelines, our office will be professionally cleaned and sanitized at the end of every working day. Our instrument sterilization process will continue to be monitored by an outside company to assure safety. Our staff is undergoing extra training to safely work in our new environment, and will be given the the highest quality hospital-grade protections (PPE) recommended by all regulatory bodies. The staff and doctors will also undergo daily temperature checks and screening questions to monitor our own health. Please do not hesitate to contact our office to discuss any questions you may have about our new health and safety policies. We sincerely look forward to seeing you again very soon. Thank you so much, stay healthy and well, Dr. Kanabar, Dr. Choi, and team https://orthodontistdallastx.com/AAOICSuppConsentCombined.pdf (Please PRINT and SIGN these two short consent forms, and bring to your next appointment) ORTHODONTISTDALLASTX.COM orthodontistdallastx.com

Hello Dear Patients, In light of recent developments regarding Coronavirus (COVID-19), and for the safety of our patients, team members, and their familes, we have decided to temporarily close the office for immediate patient care for the next 2 weeks, starting this Monday March 23rd until Sunday April 5th. We are open tomorrow and Saturday for any last non-doctor appointments needed such as delivering extra aligners/retainers, and simple appointments like wire clipping, etc. Please call our office at 214 692 5688 if you need extra elastics, wax, or need to have a wire clipped, or pick up extra aligners, etc. We can also arrange shipping via UPS if needed. We are currently calling all those patients affected in the next 2 weeks. We will be answering our phones and checking our messages from home, so please feel free to call the office line during the next 2 weeks if needed. We hope you all understand that we all can make a difference by socially distancing ourselves during this very crucial time. Please be safe and healthy. We are praying for a swift recovery phase for our country and the world. Sincerely, Your team at Walnut Central Orthodontics
