Many women request mastectomy and reconstruction all in one stage without further surgery. This can be done, but often it is helpful to stage your reconstruction for the best possible outcome. For example, if you are at risk for healing problems or skin loss with mastectomy, require additional treatment such as radiation, or desire to alter your breast size.

This young breast cancer patient treated with bilateral mastectomy desired a fuller breast and wanted the natural result achieved by using her own tissue. She was perfect candidate for a hybrid procedure, combining a DIEP flap with a small breast implant. To achieve a larger skin envelope, she was staged first with a tissue expander and followed by DIEP flap reconstruction and immediate implant placement. She is just 3 weeks out from surgery here and we can’t wait for her final result.

For patients who are thin or don’t have a significant donor site option, this an excellent method to achieve a natural result.

Our last post focused on an MRI image of the outer thigh region. That type of image is critical for planning a lateral thigh flap breast reconstruction. This patient desired to use excess tissue of the outer thigh for immediate breast reconstruction. This allowed her to eliminate dreaded “saddlebags,” a troublesome area for many women who have more body fat deposited on the thigh.

Successful harvest of this tissue is dependent on careful pre-operative mapping of the source blood vessels visualized with a MRI or CT scan.
•
Ultimately, the tissue is transferred to the breast for a natural reconstruction. The thigh is additionally contoured with liposuction for the best final result.
•
This is not the best option for every patient as scars are visible on the outer thigh. It is an excellent option for women who desire natural tissue reconstruction but do not have available donor sites or who desire an improved contour of the thigh, as for this patient.

Did you know that radiology is an essential tool for successful natural tissue breast reconstruction? This image was created by an MRI, targeting the thigh region. Depending on your body type, tissue from the abdomen, inner or outer thigh, buttock of back can be used to reconstruct the breast.

This image was performed specifically in preparation for use of outer thigh tissue in the region of a “saddlebag.” This is a less common donor site for natural tissue reconstruction but may be reasonable for patients who are unable to use the abdomen due to previous surgery or unique body habitus.

Knowledge of anatomy and unique blood supply to the outer thigh is critical for this reconstruction. Pre-operative planning with an excellent radiology team makes this possible!

This patient tested positive for a BRCA gene mutation, putting her at high risk for future breast cancer. She chose to undergo a #BilateralMastectomy with implant-based reconstruction.
Since she had a moderate amount of asymmetry and sagging of the natural breast, we decided to perform a #BreastLift first, which allowed the skin envelope and nipple-areola complex to be lifted. This was followed by nipple-sparing mastectomy and implant reconstruction with fat grafting. The patient also used 355cc High Profile Smooth Round Silicone Implants to restore fullness.
