October 2nd, 2020 Our residents and employees were tested for COVID-19 on 09/29/20. We have had one additional staff member test positive for COVID-19. We will be testing again on 10/06/20. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). The coming cooler months will continue to challenge us in limiting the impact of COVID-19 on our community. It is important for each of us to think about how our actions affect others we come in contact with. We must protect one another. You are a key partner in the safe reopening of our care center, please remember to social distance, wear a mask, and practice proper hand hygiene. September 25, 2020 Our residents and employees were tested for COVID-19 on 09/22/20. All results have been received and are shown in the chart below. We had 1 staff test positive for COVID-19. We will be testing again on September 29th. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). Dear Family and Friends, Although not unexpected, as of 09/16/2020, we have 2 staff members in our care center who have tested positive for COVID-19. This event triggers a planned process of isolating residents to aggressively minimize transmission to others. Our top priority is to keep everyone safe and healthy. We are collaborating with our Medical Director and the Minnesota Department of Health and are following all recommendations from the Centers for Disease Control & Prevention and the Centers for Medicare & Medicaid Services to ensure we are taking all appropriate steps at this time. With the confirmed positive case of COVID-19 in our care center, we will move forward with entire site testing for both residents and staff on 9/22/20. We understand that this decision may result in increased confirmed cases, but the results will allow us to accurately isolate the virus and prevent the spread in our community. We believe this is the best way to protect residents, staff and our community and track the path of the virus. This means that we must cancel all outdoor and open window visits until further notice. We will continue to use technology and closed window visits for resident connection with you. In the event we have any residents test positive, we would activate our COVID unit. The unit is segregated from the rest of our community. The designated staff that care for residents in this unit remain within the area and do not have physical contact with other non-segregated residents. Any staff who test positive will be self-quarantined at home and will not be allowed to return to work until they recover from the virus; 10 days following positive test and 72 hours from last fever without using medication. Our staff are equipped with specific training and PPE to continue to provide safe, quality care, services and support to the residents of our community. As we’ve shared several times, in early March, we initiated proactive infection control practices, screening procedures and visitor restrictions that will help us manage this situation as effectively as possible and protect the safety and health of all who live and work here. Last week we learned about the new Centers for Medicare and Medicaid Services (CMS) testing requirements that include routine staff testing. Based on our current county positivity rate provided by CMS, our care center is in the low category. CMS recalculates the county positivity rate weekly and we will keep you updated if our category changes. This category requires us to routinely test staff monthly. Our first round of routine testing for staff will be determined. We continue to test symptomatic residents and staff as symptoms arise and perform entire-site testing during an outbreak (positive case in our care center). Should a confirmed case come from any of these tests, we would promptly notify our residents, staff and their families. The threat of this devastating and deadly virus is still with us. Minnesota is experiencing an increase in the number of positive cases in areas across our state, which can pose a greater risk of exposure to both our residents and our staff. As of 9/14/20, we have no confirmed cases of COVID-19 in our care center. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). We continue to offer outdoor visits, window visits and virtual visits. As of 9/4/20, we have no confirmed cases of COVID-19 in our care center. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). This week, CMS and MDH released new testing requirements for our care center’s staff and residents that are effective immediately. “Facility Staff” is defined broadly, to include employees, consultants, contractors, volunteers, and caregivers who provide care and services to residents on behalf of the facility, and students in the facility’s nurse aide training programs or from affiliated academic institutions. The requirements outline routine testing depending on our county positivity rate: • Low: Monthly testing in counties with less than 5% positivity • Medium: Weekly testing in counties with 5-10% positivity • High: Bi-weekly testing in counties with greater than 10% positivity The rule also requires testing in three scenarios: (i) symptomatic individual identified; (ii) outbreak (new case arises in facility); and (iii) routine testing. While CMS does not recommend routine testing of residents unless the resident leaves the facility regularly, it does require routine testing of staff. As providers of senior care, we know how important family and friends are in supporting the physical and mental health of our residents. We continue to review our visitation policy on a weekly basis. In the meantime, we welcome patio visits, open and closed window visits, and virtual visits. As a reminder, you can help limit exposure in our community through appropriate hand hygiene, mask use, and social distancing. October 10, 2020 Our residents and employees were tested for COVID-19 on 10/06/20. All resident’s tested negative for COVID-19. We had at least one additional staff member test positive. We will be testing again on 10/13/20. We continue to follow all recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS). The Minnesota Department of Health (MDH) is currently developing guidelines based on the recently issued CMS guidance on how we our can safety re-open our setting to visitors and continue to protect the health of our residents and staff. We plan to revise our current visitation policies to accommodate the MDH and CMS changes as soon as possible and distribute to residents and families. We appreciate your patience as we take time to evaluate and implement the guidance so that we can safely resume indoor visitation. These requirements not only apply to our ability to resume indoor visitation but also identify triggers for when we may need to restrict visitation. Restricting visitation may happen if we have a positive case of COVID-19 among our residents and staff (what we are currently experiencing) or if our broader community falls into a higher risk category. We deeply appreciate the sacrifices you continue to make to limit the impact of COVID-19 on our community and your patience as we prepare our setting to host safe indoor visitation. Dear Family and Friends, We conducted entire-site testing (outbreak testing) on 10/13/20. All resident’s tested negative for COVID-19. We had one additional staff member test positive however, this employee had not worked in the building for over one month. In total we have had five employees test positive for COVID-19, three have recovered and have returned to work. We will be testing again on 10/20/20. We are very excited to share the news that we will soon be able to resume indoor visits, and warmly welcome you back into our care center. The Minnesota Department of Health (MDH) has issued guidance on how we can safely re-open to visitors while continuing to protect the health of our residents and staff. This guidance takes effect on October 17th. In the meantime, we remind you to take advantage of outdoor visits, window visits, and virtual visits. Even though we are excited to open up our care center for indoor visits, it is tempered by the fact that rural communities are seeing more outbreak in recent weeks which has the potential to impact our care center’s ability to offer visitation. There can be no new onset of COVID-19 cases in the last 14 days and facility cannot currently be conducting outbreak testing. COVID-19 county positivity rates will determine how our care center will facilitate indoor visits. For your planning purposes, please know we expect to accommodate indoor visitation very soon, following these strict guidelines: • All indoor visits MUST be scheduled in advance. • Visitors must be screened prior to entering the care center for signs and symptoms of COVID-19. • All visits must be conducted using social distancing. • Visitors must wear a mask covering both nose and mouth while in the care center. Residents must wear a surgical mask during the visit. • Proper hand hygiene must occur (use of alcohol-based hand rub) throughout visit. • Visits may be restricted to a designated area within the building. • Physical contact (hugs, kisses, handshakes, etc.) continues to be restricted to protect everyone from potential virus transmission. • Visitors will be limited to two visitors per resident; the total number of visitors in the building will be limited; and the duration of visits will be limited. • Visitors must adhere to all infection control policies (ICP) and staff should provide monitoring for those who may have difficulty adhering to ICP • Cleaning/disinfecting high frequency touched surfaces and designated visitation areas must occur after each visit. • All items brought to the visit must be left with staff for disinfection and not passed to the resident during the visit. Our ability to resume visitation would not have been possible without the incredible commitment and dedication of our staff, who have worked tirelessly to manage the risks of COVID-19 and providing compassionate care and support to our residents at a time when they are physically separated by their loved ones. Dear Family and Friends, Viewcrest Care Center conducted entire-site testing for both residents and staff on 10/20/20. This week we had one resident and one staff member test positive for COVID-19. We will be testing again on 10/27/20. Suncrest Assisted Living continues to have no known cases of COVID-19. The positive resident requires us to open our COVID unit. As a reminder, staff that work in our segregated unit will not care for residents outside of the unit or interact with any employees that are providing direct care to residents outside of the unit. All staff in the segregated unit will be equipped with N95 masks, face shields and gowns. Staff will enter and exit the segregated unit from a separate door to prevent exposure to the rest of our care center, residents and employees. Our assisted Living, Suncrest, will be resuming indoor visitation as of 10/19/20. As a reminder, we will follow state and federal guidance for visitation, which includes following the core infection control protocols, including screening of all visitors for signs & symptoms of COVID, proper hand hygiene, face coverings, and social distancing. Please recognize that this virus is still spreading throughout our state and that we may need to restrict visitation if we experience positive cases of COVID-19 in our setting, identify a broader risk of exposure both in our setting and/or the broader community, or encounter other factors that affect the safety of visitation. November 23, 2020 Family and Friends, Duluth Health Services regrets to inform you that we had a staff member of Viewcrest test positive for COVID-19 on 11/22/20. Point Prevalence Testing (PPT) will continue this week on Monday and Wednesday. We are excited to announce that we no longer have any residents with active COVID. We are closing our COVID unit as of today, November 23rd. As a reminder any staff who test positive will be quarantined at home and will not be allowed to return to work until they recover from the virus, are symptom free and pass the employee screening protocols. Our staff are equipped with specific training and personal protective equipment (PPE) to continue to provide safe, quality care, services and support to the residents of our community. As we continue to fight against COVID-19, we ask that you not take your loved ones out this Thanksgiving. By not taking your loved one out for a holiday visit, you will help us in protecting the health of everyone who lives and works here. One of the best things that you can do for your family this holiday season is to not gather in groups and keep everyone safe by wearing a facemask when you do. We know just how special the holiday season can be and spending time with family is what adds meaning to our celebrations. However, we need your help to protect our residents, our staff, and yourself during this pandemic’s risky time by limiting our contact with the broader community. As Governor Walz told us the other night, the COVID-19 surge is at a dangerous level in Minnesota. This past year has been a challenge like no other and we deeply appreciate the sacrifices that you and your family have already made during these past months. The hope of a COVID-19 vaccine is promising, but until that time we all need to use extra caution to slow the spread of this highly contagious disease. If a resident does choose to go to a holiday gathering outside of our care center, the resident will need to quarantine in his or her room for 14-days after returning as a necessary precaution. Even when being very careful and following all of the guidelines, the risk of being infected when being out in the general public is high. Our staff is committed to keeping you connected during the holiday seasons. Dear Family and Friends, Viewcrest and Suncrest tested all residents and staff for COVID-19 this week. The results are shown below. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). • Viewcrest –Residents were tested on Monday this week and staff were tested on Monday and Thursday. No positive cases were noted. No indoor visitation at this time. Testing staff twice weekly due to county positivity. • Suncrest – Tenants and staff were tested on 11/27 & 12/4. One additional employee tested positive. No indoor visitation at this time. Tenant and staff testing will continue weekly until 14days have elapsed with no positive cases. The CDC’s Advisory Committee on Immunization Practices (ACIP) held an emergency meeting this week to review allocation recommendations for vaccines in phase 1 distribution. The advisory committee voted to include residents of long-term care communities in Phase 1A of vaccination. We anticipate a vaccine being available in the coming weeks and will keep you informed. To unpack it, let’s go back to when God created the world. Imagine you’ve been brought to life by the breath of God, and as God pulls you from the ground, He smiles at you. From then on, whenever He looks at you, His face lights up. Life’s perfect until one day, you do the only thing God asked you not to do, and because of this act of rebellion, you separate yourself from the One who created you. In an instant, what was meant to be eternal closeness with God becomes eternal separation from Him. That’s what happens in Genesis 1-3…BUT THANKFULLY, it’s not the end of the story. Even though humanity messes up, God makes a promise that He will send someone to mend our brokenness and restore our relationship with Him. Christmas marks the arrival of God’s promised answer when the Creator God took the form of a baby and became “God with us.” Isaiah 9:6-7 tells us that “unto us a child is born, to us a son is given,” and this child was Jesus. This Wonderful Counselor and Prince of Peace is the Savior of the world that people had been waiting for. When He came to earth, prophecies were fulfilled. Jesus grew up to be a man who lived a perfect sinless life. He died on a cross and rose from the dead, granting us the opportunity to be delivered from our sins. Because of Christmas, now anyone who chooses to believe in Jesus can experience a permanent joy that only comes from knowing Him personally. Dear Family and Friends, Viewcrest tested all residents and staff for COVID-19 this week. Suncrest has completed point prevalence testing with no additional cases noted. The results are shown below. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). • Viewcrest – We currently have 12 resident’s residing on our COVID segregation unit. We have had four additional resident’s test positive this week. In addition we have had ten additional positive staff members. No indoor visitation at this time. Testing staff twice weekly due to county positivity rates. In addition we continue to test all residents and staff using our “rapid” testing methods if any signs or symptoms of COVID arise. We urge you to practice social distancing, mask use, and staying home when possible. Please refrain from taking your loved one home for the holiday. If a resident does choose to go to a holiday gathering outside of our care center, the resident will need to quarantine in his or her room for 14-days after returning as a necessary precaution. Staff are also asked to only celebrate with members of their household and to follow all quarantine guidelines after traveling. We want to wish all our friends and family a Merry Christmas. We have had a week full of activities on our care campus including celebrating the twelve days of Christmas along with a special visit from Santa today! We are still waiting some results from the testing completed on December 28th and 30th. Should any additional positive cases occur, we will notify all residents, staff, and family representatives promptly. Our next round of testing will be conducted on January 4th. The residents that tested positive for COVID-19 have been transferred to our COVID unit. As a reminder, staff that work in our segregated unit will not care for residents outside of the unit or interact with any employees that are providing direct care to residents outside of the unit. All staff in the segregated unit will be equipped with N95 masks, face shields and gowns. Staff will enter and exit the segregated unit from a separate door to prevent exposure to the rest of our care center, residents and employees. Due to the impact on staffing from the COVID-19 virus, the Minnesota Department of Health (MDH) has coordinated with the National Guard to provide us with staffing relief starting Saturday, January 2nd. Dear Family and Friends, As of today, Suncrest has no confirmed cases of COVID-19. Viewcrest has had 8 additional residents test positive for COVID-19 within the last 24 hours and 3 additional staff members test positive for COVID-19 within the last 24 hours. As of today we have received testing back from Monday, 12/21/20 and some from Wednesday, 12/23/20. We are currently awaiting test results from Monday, 12/28/20 testing. Our care center continues to test residents and staff twice weekly. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). We are looking forward to receiving the Moderna COVID-19 Vaccine in the near future. The second dose of the vaccine will be administered 4 weeks after the first round and the third will be administered 4 weeks after the second. Protection from vaccine is not immediate; vaccine is a 2-dose series and will take 1 to 2 weeks following the second dose to be considered fully vaccinated. Vaccine efficacy is 94-95%. This data was assessed with two doses being administered. It is very important to get both doses to ensure good protection. Assisted living vaccination dates will be sent out soon. When someone receives the vaccine they may experience side effects up to three days after vaccination. These side effects include injection site pain, tenderness and swelling of the lymph nodes in the same arm as the injection, swelling (hardness), and redness. General side effects include fatigue, headache, muscle pain, joint pain, chills, nausea, and low grade fever. Getting vaccinated against COVID-19 is one of the best ways to protect yourself and everyone around you, particularly those who are at increased risk for severe illness. Wearing masks and social distancing help reduce your chance of being exposed to the virus or spreading it to others, but these measures are not enough. Getting the vaccination will not only protect you from being infected; it will help us reduce visitation restrictions in the future. While the vaccine is a significant factor in our fight against the virus, it is just the beginning to reopening our care centers. We continue to use the county positivity rate to measure the spread of the virus in our local communities and follow the recommendations from MDH and CDC on reopening. Decorations will come down and ugly Christmas sweaters will be stowed away, but the warmth Christmas time brings can remain. We just have to make an effort to be the people from January through October that we find ourselves being in November and December. The most powerful thing we can do to keep our hearts alive and thriving year round is to cultivate a heart of gratitude. Our care center will begin monthly testing next week. The CDC released a report that indicates a national average vaccination rate for care center residents of 77.8% and of care center staff, 37.5%. Minnesota is in a category with higher rates for both, with more than 80% of residents and more than 40% of staff. As of Wednesday of this week, 70,049 residents and staff of Minnesota’s long term care centers have received at least one dose of the COVID-19 vaccine. We have conducted our second vaccine clinic giving many residents and staff their second dose of the Moderna vaccine! We will be hosting our 3rd Clinic in March. Our care center will begin monthly testing next week. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). As of today, the MDH is not recommending any changes to our current visitation process. Since the start of the pandemic, 3,977 residents of Minnesota's long term care facilities have died from COVID-19, accounting for 63% of the deaths from the virus statewide. However, last week, Minnesota recorded an average of just 64 new coronavirus cases per day in long-term care settings; the lowest the state has experienced since September! On Tuesday, MDH released more data showing that overall trends are improving, and not just in senior services communities. The state's positivity rate of testing has fallen to 4%, the lowest rate since June! The rate had peaked at 15.5% in November amid an upsurge in cases. The drop in new cases is a hopeful sign that the vaccines are starting to provide the protection we need and the infection control protocols put in place are working. Our staff continue to find creative ways to ensure social distancing does not stop social interaction. This week we plan to celebrate Valentine’s Day by with a resident party. This Saturday, March 6th, marks one year since Minnesota confirmed its first case of COVID-19. The state has since recorded 485,230 total confirmed or probable cases. Now, more than 52% of Minnesotans 65 and older have received at least one dose of either COVID vaccine. This week, Governor Tim Walz stated Minnesota will expand vaccination eligibility when 70% of that population gets their first dose. Vaccine eligibility would expand to people with high-risk health conditions and targeted essential workers. Younger groups and the general public would follow in the late spring and summer. Johnson & Johnson has now received an emergency use authorization (EUA) from the U.S. Food and Drug Administration (FDA) for its COVID-19 vaccine, the Janssen Ad-Vac. The J&J vaccine is different than its Moderna and Pfizer 2-dose vaccine counterparts in two ways. First, the J&J vaccine is a one-shot vaccine, not a two-dose vaccine. Second, the J&J vaccine uses a more common vaccine technology and does not utilize the mRNA vaccine delivery method. The vaccine is published as 85% effective 28 days post vaccination. It is not necessary to sign up if: • You have already received your COVID-19 vaccine. • You previously registered for the COVID-19 Vaccine Pilot Program or the COVID-19 Community Vaccination Program. You’re already on the Vaccine Connector list, and there is no need to re-submit your information. • You completed the Minnesota COVID-19 Vaccine Education and Child Care survey. You’re already on the Vaccine Connector list, and there is no need to re-submit your information. • You completed the Health Care Connection Form, you do not need to re-submit your information. The Minnesota Department of Health will work with health systems and local public health agencies to notify you about when and where you can get vaccinated. We look forward to the day we can throw our doors wide open to the community. Until then, there are multiple ways residents in our setting can interact with one another and with their loved ones. We have resumed socially distanced communal dining and small group activities. We are currently able to offer indoor visits, virtual visits, essential caregiver visits, and compassionate care visits. We do ask that visitors are tested prior to entering our care center. Due to cost, we will no longer be able to test essential caregivers, compassionate caregivers or visitors in the care centers. Dear Family and Friends, As of today, we have no new confirmed cases of COVID-19 in our care center and no confirmed cases of COVID-19 in Suncrest. Our care center will begin monthly testing next week. We are following recommendations from the Minnesota Department of Health (MDH), the Centers for Disease Control & Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). As of today, the MDH is not recommending any changes to our current visitation process. Currently, vaccines are being distributed to adults 65 and older, educators, school staff, child care workers, health care workers, and residents and staff of long-term care facilities. We have received many calls requesting information about vaccinations. If you qualify in one of the above categories, please check the state’s online vaccine locations map (https://mn.gov/covid19/vaccine/find-vaccine/locations/index.jsp) or contact your health care provider about appointment availability. Our staff and residents continue to have opportunities for on-site vaccinations through our pharmacy partnership. Our next clinic is on date. Minnesota continues to see new COVID-19 cases and hospitalizations as MN lessons the restrictions in public areas. The numbers of cases and attributable deaths are declining as vaccinations roll out. State and local health officials continue to stress social distancing, mask use, and frequent hand washing or sanitizing. With vaccinations becoming more prevalent, and knowledge that vaccinations are providing a positive impact in reduction of active outbreaks; it is frustrating that use of PPE and visitation constraints must continue within our care centers. Until the CDC and MDH have supporting science to assure the spread of COVID-19 is reduced with vaccinations, we are required to maintain the same infection control protocol. Looking for ways to help?
